Your first visit and what to expect
Plan on 45-60 minutes for your first visit, which will include an interview and a physical examination. The length of the exam depends on the complexity of your symptoms. Time permitting, your treatment will begin immediately following the therapist’s assessment.
If you have not received paperwork in advance (via internet, mail or fax), please arrive 15 minutes prior to your scheduled appointment so that you have time to complete your paperwork before you see the physical therapist.

What to wear
Comfortable, loose clothing (t-shirts, shorts, sweats). If you are being evaluated for orthotics, please bring a selection of shoes that you wear regularly, as well as shorts or pants that can be rolled above the knee. Shorts can be provided for you if you forget to bring them.
Please refrain from wearing perfume or cologne at your visit. Some individuals are highly sensitive, and can have an allergic reaction.
What to bring:
1. Your insurance card, claim number, and/or billing information.
2. Your physician referral for physical therapy, if required by your insurance.
3. Your completed registration and health history forms (available online below, or by mail or fax).
4. Any applicable copay.
Treatment of Minors
Clients under 18 years old must be accompanied by a parent or guardian on their first visit. The authorization to treat forms must be signed before treatment begins.
